Annual Physicals For 2026-2027 School Year🐾
Date: Tuesday, April 28th
Time: Doors will open at 5:30 p.m. and will close at 7:30 p.m.
Where: Midway High School Arena
Who: Any student (incoming 7ᵗʰ- 12ᵗʰ graders) planning on participating in Midway Athletics during
the 2026-2027 school year.
Price: $20.00
Please bring the UIL Medical History and Physical Examination page with you on the day of physicals. Forms MUST BE signed by a parent. Make checks payable to Midway Athletics. Forms can be found at http://midwayisd.rankone.com
Physicals will be performed on a FIRST COME-FIRST SERVE basis. This will be for one night only.
