Athletic Physicals

All students planning to participate in athletics during the 2024-2025 school year must have a physical on file with the school that is dated after April 1, 2024.

Midway High School is offering an opportunity for students to obtain a physical for $20 on April 30 at the High School Arena from 5:30 - 8:30 pm.

Physical forms must be completed and signed by a parent.  Those forms can be found at

Checks can be made out to Midway Athletics. We will not accept credit card or Venmo.

Students athletes must have a physical and Rank One paperwork completed prior to participating in Strength & Conditioning Camp.
