Transition services are activities that prepare students with disabilities to move from school to post-school life.  The activities must be based on the student’s needs, preferences, and interests, and shall include needed activities in the following areas:

    1. Instruction
    2. Related services
    3. Community experiences
    4. Development of employment and other post-school adult living objectives
    5. Acquisition of daily living skills (when appropriate)
    6. Functional vocational evaluation

It's never too early for parents to think about transition. Transition plans should actually drive the IEP goals so that students are prepared to the fullest extent possible.

The Texas Transition and Employment Guide provides resources to help young people and their parents prepare for successful transition toward education, employment, military, or independent living. This guide is a living, working online tool that stay current with input from legislation, parents, students, and other stakeholders.

The online English and Spanish versions of this Guide are found here
