MISD Pre-Participation Packets for the 2012-2013 school year are now available.  All athletes must have a new packet on-file with the campus athletic trainer prior to any athletic participation. 

2012-2013 Pre-participation packets can be printed from the link above or YELLOW PPE Packets can be picked up from your campus athletic trainer or from the front office of your campus.

Pre-participation exams must NOT be dated prior to 4-1-2012 to be considered valid for the 2012-2013 school year.  Packets must be turned DIRECTLY into the athletic trainer on your campus.
