You may access Employee Access Center regardless of whether you are employed with the district or not. Please keep your address and email address updated in Employee Access Center. This ensures that tax information goes to the correct location at the end of the year. Information on this request form does not move over to your address and email address in other systems.

I understand that the service record has not been signed by me. I understand that two copies will be sent to the receiving district, and it is my responsibility to sign both copies, verifying the information on the record. One of the signed copies will be mailed back to Midway ISD for their records. If this is not acceptable, I understand that I will need to sign the service record at the Midway ISD Administration building prior to the documents being released. I also understand that service records can not be processed until the end of June for the current year as leave information is still being processed.
